Care Packages

1. Pre-Discharge Coordination – Contact us a day before discharge (or as soon as possible), and our clinical team will visit the patient’s home to conduct a detailed care needs assessment, evaluate equipment requirements, and create a personalized recovery plan.

2. Hospital Takeover & Safe Transfer – On discharge day, our trained caregiver will meet you at the hospital, coordinate with the medical team, and safely bring the patient home.

3. Integrated Recovery Support – Once home, we provide physiotherapy for mobility, speech & swallowing therapy for safe eating and communication, and occupational therapy to regain daily independence. Our dietitian-designed meal plans, vitals monitoring, and medical nursing care (wound or pressure sore care, medication management) ensure holistic healing.

4. Specialized Care – For patients with feeding tubes (Ryles) or tracheostomies, we offer expert management, while family training equips caregivers with skills for long-term support.

🟢 Basic Recovery Package

(For stable patients needing essential support)

Daily nursing care (vitals, medication)
Physiotherapy – 3 sessions/week
1 Diet consultation (basic meal plan)
1 Family caregiver and prevention training session
Home safety & equipment assessment
